ABSTRACT

ABSTRACT Pyogenic granuloma is a common benign, vascular lesion of the skin and mucosa. Our case was a 34-year-old woman at 28 weeks of gestation. She presented with a complaint of a growing and occasionally bleeding mass on the left lower eyelid conjunctiva for approximately 3 months. On examination, a red, pedunculated fragile lesion on the medial lower eyelid conjunctiva was observed. Ophthalmologic examination revealed that visual acuity was 20/20 in both eyes. Anterior and posterior segment examination was normal bilaterally. The lesion was excised completely under local anesthesia and sent for pathological examination. Histopathological examination revealed multiple vascular structures with small-diameters in the fibrous stroma. Vessels showed strong CD31 expression in immunohistochemical staining. Kaposi sarcoma was excluded due to negative HHV-8 staining. These findings were diagnostic of pyogenic granuloma. The frequency of pyogenic granuloma increases during pregnancy and surgical excision is important in diagnosis and treatment of these patients. This is the first reported case of conjunctival pyogenic granuloma during pregnancy.


RESUMO O granuloma piogênico é uma lesão vascular benígna comum da pele e da mucosa. Neste caso, uma mulher de 34 anos com 28 semanas de gestação, apresentou queixa de massa em crescimento e sangramento ocasional na conjuntiva inferior da pálpebra esquerda por cerca de 3 meses. No exame, detectou-se uma lesão frágil, pedunculada vermelha na conjuntiva na porção medial da pálpebra inferior No exame oftalmológico, a acuidade visual foi 20/20 em ambos os olhos. O exame dos segmentos anterior e posterior foi normal bilateralmente. A lesão foi excisada totalmente com anestesia local e enviada para exame patológico. No exame histopatológico, múltiplas estruturas vasculares de pequeno diâmetro foram observadas no estroma fibroso. Os vasos apresentaram forte expressão de CD31 na coloração imuno-histoquímica. O sarcoma de Kaposi foi excluído devido à negatividade da coloração do HHV-8. Esses achados foram diagnósticos para granuloma piogênico. A frequência de granuloma piogênico aumenta durante a gestação e a excisão cirúrgica é importante no diagnóstico e tratamento desses pacientes. Este é o primeiro caso relatado de granuloma piogênico conjuntival ocorrido durante a gestação.

ABSTRACT

Abstract Lobular capillary hemangioma or pyogenic granuloma is a benign vascular tumor of the skin or mucous membranes. Most patients present a single lesion. It manifests clinically as an erythematous, friable, and fast-growing tumor. This report details a case with exuberant presentation in a patient with ankylosing spondylitis, using adalimumab. Factors triggering pyogenic granuloma are not well known. They may spontaneously regress, but most require treatment.

ABSTRACT

A nine-year-old girl with of juvenile idiopathic arthritis (JIA) in use of methotrexate complained of a self-bleeding ulcerated lesion exhibiting a red surface in the lower lip. Pyogenic granuloma was confirmed by histopathological analysis. After the 7th and 15th days, the lip where biopsy had been done exhibited a deficient healing. The case was discussed with the physician for temporary suspension of methotrexate. Complete wound healing of the biopsied site occurred after temporary interruption of the medication. Wound healing after methotrexate temporary suspension allowed concluding that the drug delayed surgical wound healing was a consequence of drug cytotoxicity. (AU)


Uma menina de nove anos de idade com artrite idiopática juvenil (AIJ) em uso de metotrexato queixou-se de lesão ulcerada com sangramento espontâneo que exibia superfície avermelhada em lábio inferior. O granuloma piogênico foi confirmado pelo exame histopatológico. Após o 7º e 15º dias, a região do lábio onde a biópsia foi realizada exibiu cicatrização deficiente. O caso foi discutido com o médico e foi realizada a suspensão temporária do metotrexato que resultou em cicatrização completa. A cicatrização da ferida após a suspensão temporária do metotrexato permitiu concluir que o medicamento retardou a cicatrização da ferida cirúrgica como consequência da citotoxicidade da droga. ABSTRACT

Fundamento: el hemangioma piógeno o granuloma piógeno, incluye neoplasias que se caracterizan por un gran número de vasos sanguíneos normales o anormales que resultan difíciles de diferenciar de malformaciones o hematomas. Es una tumoración de tejidos blandos, excepcionalmente en el tejido óseo, siendo relativamente frecuente, resulta de un desarrollo reactivo focal de tejido fibrovascular con proliferación endotelial y se les relaciona con traumas, irritación, cambios hormonales, el embarazo. Objetivo: describir la localización infrecuente y la evolución de la enfermedad. Caso clínico: paciente blanco masculino antecedentes de notar una lesión de aspecto granulomatoso en cara interna del trago, por cuyo motivo se incindió con anterioridad y por el crecimiento rápido se decidió exceresis total y biopsia, se informó la presencia de un hemangioma piógeno. Conclusiones: el hemangioma o granuloma piógeno, es una tumoración benigna de inicio brusco y de curso rápido. La irritación local de los tejidos es la causa más frecuente y el tratamiento quirúrgico es la terapéutica adecuada para la curación de la enfermedad.


Background: pyogenic hemangioma or pyogenic granuloma includes neoplasms characterized by a large number of normal or abnormal blood vessels that are difficult to differentiate from malformations or hematomas. It is a soft tissue tumor, exceptionally in bone tissue, being relatively frequent, results from a focal reactive development of fibrovascular tissue with endothelial proliferation, and is associated with trauma, irritation, hormonal changes, pregnancy. Objective: to describe the infrequent location and the evolution of the disease. Clinical case: a white male patient had a history of a granulomatous lesion on the internal side of the swallow, for which reason it was previously incinerated and for rapid growth it was decided total exceresis and biopsy reporting the presence of a pyogenic hemangioma. Conclusions: pyogenic hemangioma or granuloma is a benign tumor of rapid onset and development. Local tissue irritation is the most common cause and surgical treatment is the appropriate therapy for the cure of the disease.

ABSTRACT

Abstract Pyogenic granuloma (PG) is a benign vascular lesion of the skin and mucous membranes, presenting as a solitary, red, pedunculated papule that bleeds easily and which appears to be a reactive vascular proliferation to minor injury. These lesions are more common in children and are mostly seen in exposed sites. Multiple disseminated PG is a rare condition. This report describes an infant with multiple, disseminated, pyogenic granulomas following burns from boiling milk. We reviewed literature published in English and found 9 cases. Six cases were provoked by hot milk, 2 by hot water, while the cause of the other case is unknown. Angiogiogenetic activity in milk may explain PG development in these patients.

No todas las formaciones que se encuentran asociadas a la onicocriptosis y localizadas en el labio ungueal son granulomas piogénicos, ya que en la mayoría de las ocasiones se trata de una proliferación fibrosa fruto del afán reparativo del organismo. Por tales razones se decidió la presentación del caso de una paciente con onicocriptosis asociada a granuloma piógeno, diagnosticada con anterioridad en el Servicio de Ortopedia del Hospital General Universitario Dr. Gustavo Aldereguía Lima de Cienfuegos. Esta paciente acudió a la consulta de Podología de la Filial de Ciencias Médicas, por presentar recidiva. Se determinó aplicar la técnica de cirugía menor fenol/alcohol, con resultados positivos.


Not all growths associated with onychocryptosis and located on the ungueal labia are pyogenic granulomas, since in most cases they are fibrous proliferations caused by the reparative response of the body. Hence, we decided to present the case of a patient with onychocryptosis associated with pyogenic granuloma diagnosed earlier in the Orthopedics Department of the Dr. Gustavo Aldereguía Lima University General Hospital of Cienfuegos. This patient attended the Podiatry consultation of the Medical Sciences Subsidiary Faculty due to recurrence. Phenol and alcohol procedure was performed with positive results.

ABSTRACT

Objective: An epidemiological survey of oral soft tissue proliferative processes diagnosed at the center of Pathological Anatomy of Oral ASCES Faculty. Methods: We analyzed all the histopathological reports in the period from 1999 to 2009 as regards the following variables:histopathologic diagnosis, sex, age, symptoms, type of biopsy and location of the lesion. Results: Non-neoplastic proliferative processes were present in 328 of the 938 reports analyzed, reaching a prevalence of 35 %. The most common injury was fibrous hyperplasia (81.4%) followed by pyogenic granuloma (11%) and peripheral giant cell lesions (6.1%). Fibrous hyperplasia was more prevalent in females, in the age group 40 to 59 years, where the most frequently performed biopsy was the excisional type, with the most frequent location being the buccal mucosa, and in most cases, no painful symptoms were reported associated with it, demonstrating significant difference between the variables studied (p < 0.001). Conclusion: Non neoplastic proliferative processes processes are found in the general population, more common in females, affecting wide age-range, predominantly in individuals in the fifth and sixth decades of life, and fibrous hyperplasia was the most frequently found lesion.


Objetivo: ealizar um levantamento epidemiológico dos processos proliferativos dos tecidos moles orais diagnosticadas no Centro de Anatomia Patológica Oral da Faculdade ASCES. Métodos: Foram analisados todos os laudos histopatológicos no período de 1999 a 2009 em relação às seguintes variáveis:diagnóstico histopatológico, sexo, faixa etária, sintomatologia, tipo de biópsia e localização da lesão. Resultados: Os processos proliferativos não neoplásicos estiveram presentes em 328 dos 938 laudos analisados, atingindo uma prevalência de 35%. A lesão mais freqüente foi a hiperplasia fibrosa (81,4%) seguida do granuloma piogênico (11%) e lesão periférica de células gigantes (6,1%). A hiperplasia fibrosa foi mais prevalente no sexo feminino, na faixa etária de 40 a 59 anos (p< 0,001), onde o tipo de biópsia mais realizado foi excisional com a localização mais freqüente na mucosa jugal e na maioria dos casos, não foi relatada sintomatologia dolorosa associada, comprovando-se diferença significativa entre as variáveis estudadas (p< 0,001). Conclusão: Os processos proliferativos não neoplásicos são encontrados na população em geral, mais comum no sexo feminino, acometendo ampla faixa etária, predominantemente em indivíduos na quinta e sexta décadas de vida, sendo a hiperplasia fibrosa a lesão mais frequente.

ABSTRACT

As acrometástases, principalmente para as mãos, são incomuns e representam cerca de 0,0070,2 por cento de todas as lesões metastáticas. O pulmão é o sítio de origem mais comum, colaborando com 4050 por cento dos casos relatados na literatura. Os rins e mamas são outras localizações também relacionadas a neoplasias que metastatizam para as mãos, além de, mais raramente, trato gastrointestinal, outros tumores sistêmicos e sarcomas. Seu diagnóstico precoce é difícil, pois pode ser assintomático, se assemelhar a tenossinovite, artrite, paroníquia, granuloma piogênico ou infecção local. No presente relato, os autores apresentam paciente com diagnóstico de acrometástase, em ambos os quartos quirodáctilos, oriunda de carcinoma basaloide de canal anal, com pobre resposta à radioterapia.


Acrometastasis is a rare occurrence, especially when affecting the hands. It represents around 0.007-0.2 percent of all metastatic lesions. The most common site of origin is the lung, accounting for 40-50 percent of all cases reported in the literature. Kidneys and breasts are other sites also associated with neoplastic lesions that disseminate to the hands. More rarely, the site of origin may be the gastrointestinal tract or other systemic tumors or sarcomas. Early diagnosis is difficult, since the condition may be asymptomatic or may mimic tenosynovitis, arthritis, paronychia, pyogenic granuloma or a local infection. In the present paper, the authors report on a patient with the diagnosis of acrometastasis on both hands originating from a basaloid carcinoma of the anal canal. Response to radiotherapy was poor.

ABSTRACT

Este relato apresenta observação curiosa (anedótica) de paciente aparentemente hígida, portadora de corpos estranhos metálicos autointroduzidos no couro cabeludo, em decorrência de psicopatia de autoconsumo. A retirada operatória promoveu a cicatrização dos orifícios infectados, trazendo conforto à paciente.


This report presents a curious observation (anecdotal) of apparently healthy patient, which has metallic foreign bodies self applied on the scalp, due to the self psychopathy of self-consuming. The surgical removal promoted the healing of infected holes, bringing comfort to the patient.

Os autores relatam um caso de granuloma piogênico subconjuntival pós retinopexia com introflexão escleral. É uma complicação pós-cirúrgica incomum neste tipo de procedimento.

Objetivos: Avaliar a prevalência do granuloma piogênico e comparar os dados obtidos com outros relatos na literatura mundial. Métodos: O material do estudo foi levantado a partir dos registros de pacientes com diagnóstico de granuloma piogênico oral no Laboratório de Patologia Oral da Faculdade de Odontologia, da Universidade de Pernambuco, no período de janeiro de 1992 a março de 2007 (15 anos). Foram analisados os indicadores gênero, faixa etária, raça, localização anatômica, diâmetro das lesões e presença de sintomatologia. Resultados: Dentre os 5007 registros presentes no laboratório, 3,81% correspondiam a lesões diagnosticadas como granuloma piogênico oral, onde 19,9% dos pacientes pertenciam a segunda década de vida, 40,1% eram da raça branca, a gengiva foi o local mais acometido (77,9%) e lesões de menor diâmetro (0,1 a 2 cm) foram as mais observadas no diagnóstico inicial. Conclusão: As características clínico-patológicas do granuloma piogênico oral na população estudada é similar aos outros estudos presentes na literatura.


Objectives: To evaluate the prevalence of pyogenic granuloma and compare the data obtained with those of other reports in the world literature. Methods: The study material was surveyed from the records of patients with diagnosis of oral pyogenic granuloma, at the Oral Pathology Laboratory of the School of Dentistry of the University of Pernambuco, in the period from January 1992 to March 2007 (15 years). The following indicators were analyzed: gender, age group, race, anatomic location, diameter of lesions and presence of symptomatology. Results: Among the 5007 records in the laboratory, 3.81% corresponded to lesions diagnosed as oral pyogenic granuloma, in which 19.9% of the patients were in the second decade of life, 40.1% were white, the gingiva was the most affected location (77.9%) and lesion of smaller diameter (0.1 to 2 cm) were those most observed at the initial diagnosis. Conclusion: The clinical-pathological characteristics of oral pyogenic granuloma in the studied population are similar to those of other studies in the literature.
